Sports physicals have gone digital! All necessary paperwork for athletes will be completed on FinalForms next school year. On May 1st, Registration for all sports for the 2025-26 school year will be open. Once you have selected the sports your athletes wish to participate in, Finalforms will generate the necessary forms for you to complete or update. Registration will remain open until the start of each sports season.
The OHSAA Physical Examination Form signed by your doctor can either be uploaded to FinalForms to be reviewed or turned into Danny Grine in the Athletic Office.
All Athletes grades 5-12 are required to have a sports physical on file before the start of their sports season.
