Student Athlete Requirements
 
  1. All student athletes in Grades 5-12 are required to have a sports physical. Click the link below for the appropriate packet.
  2. All student athletes and parents in Grades 7-12 are required to watch two videos and complete a Verification Form for the athletes to participate. 
After watching both videos, all families must complete the OHSAA and Norwalk Catholic Rules Verification Form (click on link below) and turn into their head coach or the Main High School Office or Athletic Office. Only one completed form is needed per family. Forms must be completed and turned in by Friday, July 28th. Failure to do so will prohibit the ability of your son(s)/daughter(s) to participate in their sport.
 
The Sudden Cardiac Arrest Form is page 18 of the 2024-2025 Physical Packet (click the link below). If you have already completed and turned in the Physical Packet to the school/athletic office or coach, you do NOT need to complete the Sudden Cardiac Arrest Form again.
